Medical Visa from Kenya to Slovakia Description

This visa is for Kenyans who want to be treated for one medical condition or the other in Slovakia.
Slovakia provides good quality healthcare so it is understandable why people would want to go there for treatment.
A letter signed within the last three months by the applicant’s doctor in Kenya providing information about the medical treatment sought by the patient is required. This letter should also state why Slovakia is the best place to receive treatment for the condition.
Medical insurance with a minimum coverage of EUR 30,000 is needed.
The receiving doctor in Slovakia is also required to provide a letter detailing the nature of the treatment, estimated recovery time, the cost of treatment and the need for return visits.
A letter from your employer stating allowance and awareness of your leave is required. The term of employment and granted leave days should be stated in the letter.
Proof of paid medical fees should be provided.
Retired applicants must provide pension statements of the last 6 months.
Minors must ensure they possess the original copy of their birth certificate. If they are traveling alone or with one of their parents, a document of consent which is signed by both parents of the minor should be provided.
Foreigners with a medical visa are not permitted to work in the country.
You are expected to leave the country at the end of your treatment.

A Kenyan passport valid for at least 6 months from the intended arrival date and must have 2 blank pages for visa stamp.

If the applicant is a minor, the parents/legal guardians have to sign the form.
Passport-size photographs (35 x 45mm) taken in white background.
