Medical Visa from Kenya to Czech Republic
- This visa is for foreigners who are not in the best health condition and require medical treatment in the country.
- A medical report from your doctor in Kenya stating the type of illness and the reason the treatment should be gotten from the Czech Republic.
- Also, the doctor in the Czech Republic should provide a letter stating that the patient is accepted and the treatment can be performed. The nature of the treatment and the intended time of recovery should be stated.
- A document that shows estimated time of recovery should be sent to the embassy by your physician.
- Your employer in Kenya should provide a letter stating allowance of leave. The term of employment and the purpose of the trip should be provided.
- Information stating who will be financially responsible for the applicant during his stay in the country should be provided.
- Persons accompanying the applicant will apply for a Schengen visa under the modality Private visit
- Proof of full or part payment of medical fee should be provided.
- Proof of paid visa application fee is required.
- If you are under the age of 18 years, the original copy of your birth certificate should be provided.
- Applicants of this visa cannot enroll to study in one of the institutions of learning in the country.
- You are expected to leave the country at the end of your treatment.
- Holders are not allowed to take up permanent residence in the country.
